Our commitment
Luma Dental, PLLC is required by law to maintain the privacy of your protected health information (PHI), to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.
How we may use and share your information

Treatment. To provide and coordinate your dental care, including with specialists and labs.
Payment. To bill and get paid by you, your insurance carrier or another third party.
Health care operations. To run our practice, train staff and improve the quality of care.
Appointment reminders. To contact you about upcoming visits and treatment options.
As required by law. When federal, state or local law requires it, including public health and safety reporting.
Uses that require your permission
We will not use or share your information for marketing, or sell your information, without your written authorization. You may revoke an authorization in writing at any time, except where we have already acted on it.
Your rights
Get a copy of your records. Ask to see or receive an electronic or paper copy of your dental record.
Ask us to correct your record. Request changes if you believe information is incorrect or incomplete.
Request confidential communication. Ask us to contact you in a specific way or at a different address.
Ask us to limit what we share. Request restrictions on certain uses, including sharing with your health plan when you pay in full out of pocket.
Get a list of disclosures. Ask for a list of times we have shared your information.
Get a copy of this notice. Ask for a paper copy at any time, even if you agreed to receive it electronically.
Choose someone to act for you. A legal guardian or medical power of attorney can exercise your rights.
Our responsibilities
We will let you know promptly if a breach occurs that may have compromised the privacy or security of your information.
We must follow the duties and privacy practices described in this notice.
We will not use or share your information other than as described here unless you tell us we can in writing.
Complaints
If you believe your privacy rights have been violated, you can file a complaint with our Privacy Officer using the details below, or with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.
Changes to this notice
We can change the terms of this notice, and the changes will apply to all information we have about you. The new notice will be available on request, in each of our clinics and on this website.
